Registration is best conducted in person. Two forms of identification are required, one of which must show the registrant's address. Acceptable forms of ID include a driver's license, a utility bill in the applicant's name, an identification from the registrant's school/work or public assistance, a printed checkbook with the registrant's name and residence address, birth certificate, or mail postmarked to the registrant's current address from a federal, state, or city agency, utility, etc. Registration can be done by mail application, but the voter must vote in person, not absentee.

After receiving your application, the Clerk's office will mail you a voter identification card, which should arrive in a few weeks. You may also verify your registration at the Cook County Clerk website
